Train Collision Hermalle-sous-Huy

On 5 June 2016 a collision took place between a passenger train and a freight train at Hermalle-sous-Huy (Belgium). Three people died (including the driver) and nine people were seriously injured.

The Investigation Body for Railway Accidents and Incidents performed an accident investigation and asked Intergo to perform the study about the human and organizational factors that played a role in the accident. The study focused specifically on the actions of the driver in the run-up to the collision.

The research question focused on what human and organizational factors can explain why the train driver has not reduced the train speed in accordance with the signal order, and why he did not stop the train in accordance with the signing order.
